Prices Effective
Consultations are free. The fees indicated include computer imaging (nose reshaping & liposuction), operating room fees, surgeon's fees, anesthesia fees, implants (for surgery requiring implants), garments, pre-op and post-op visits.

Fees
The fees indicated are for the procedures listed. Any discounts are given for multiple procedures. Deviation from that plan may result in loss of discount. Dr. Mangubat's quotes are an estimate only and are subject to change at the doctor's discretion. His quotes are inclusive of all expenses required by our facility: all operating room (facility) fees, doctor's fees, implants (when applicable), garments, pre-op and post-op visits. Anesthesia fees given are an estimate only and are subject to change. Anesthesia fees are due on the day of surgery and paid directly to the anesthesia provider. Prices do not include prescription drugs, necessary laboratory fees or additional garments. Any revisions or touch ups are subject to additional facility and anesthesia fees.
Booking Surgery
A deposit equaling one half (50%) of the Doctor & Facility fees, of which $1,000.00 is non-refundable and non-transferable, is due at the time you schedule your surgery to reserve the surgery day. Surgery times are scheduled to benefit all patients as determined by the doctor and anesthesia provider. As such, your time may change so we ask that all patients be flexible with the actual time of their surgery. A deposit holds a date for surgery, not the time. The remainder of the Doctor & Facility fees is due at your pre-operative appointment or at least ten (10) business days prior to surgery, whichever date comes first. The Anesthesia fees will be due on your day of surgery.
Surgery Days
Anesthetic surgery days are Tuesday through Friday 8:00 am to 1:00 pm. Hair transplant surgery is scheduled on Tuesdays, Thursdays and Fridays. Our surgery schedule fills up quickly; please call our office as soon as you know when you'd like to have your surgery.
Types of Payment
Payments may be made to our office by cash, check, cashier's check, money order, Visa, MasterCard, Discover and American Express or through approved finance companies (Capital One, CareCredit and SurgeryLoans.com). Third party and business checks are not accepted. Personal checks will not be accepted less than ten (10) business days prior to surgery. Please check with the office for the types of payments the anesthesia provider accepts.
Cancellation Policy
A refund equal to the amount deposited minus the $1,000.00 non-refundable and non-transferable deposit will be given if surgery is canceled at least two (2) weeks prior to the scheduled surgery day. A refund equal to 50% of the refundable fees will be given if surgery is canceled 1-2 weeks (8-14 days) prior to the scheduled surgery day. If surgery is canceled within seven (7) days of surgery, no refund will be given.
